Patient Medical Records

In order to access your medical records, you will be given a security access code in the office. We will not give security codes over the phone. This will allow you to register online at Medical Office Online in which you will create your username and password in order to log in. This will allow you to access the registration page where you will enter the security code given to you in the office. If you have any trouble accessing your records, please contact our office so that we may assist you. With Medical Office Online you will be able to do the following:

• View your demographics and insurance information including your photo id and insurance cards.
• Suggest changes to your demographics and insurance information.
• View your past and upcoming appointments.
• View your medical record.
• View your current medication list.
• View your test results.
• View/Print your current statement.
• Securely message our healthcare provider or staff.
